do forever discharge and agree to hold harmless Wild Hearts Adventure Co, and its location property owners, directors, members, volunteers, and staff from any and all liability, claims or demands for personal injury, illness, including communicable disease, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred while the above-listed child(ren)  is/are participating in Wild Hearts Adventure Co’s classes, activities or events. 

I, the parent or legal guardian of the child(ren), hereby grant my permission for said child(ren) to participate fully in Wild Hearts Adventure Co. classes, events, and activities.

I understand that this release shall be legally binding until revised by Wild Hearts Adventure Co. or my child’s disenrollment from all Wild Hearts Adventure Co. classes.
